Study: your starting body composition predicts how much of your GLP-1 weight loss is muscle

A real-world study of 396 GLP-1 users found muscle made up 21.6% of the weight lost, and that your muscle-to-fat ratio at the start predicts your share.

Most women who search for a GLP-1 diet are looking for a list of things to cut. A study published on 29 July 2026 in Mayo Clinic Proceedings: Digital Health quietly argues for the opposite question: not what you remove, but what you keep, and whether the body you started with already tilts the odds.

What happened

A team led by Agathe Chabassier of Sorbonne Université, Inria and Withings matched 396 GLP-1 users against 1,635 non-users, using body composition data from connected devices people already had at home. The readings ran from June 2023 to July 2024.

Over one year the GLP-1 group lost a median 9.8 percent of body weight, roughly 10.2 kg. That figure broke into 7.5 percent fat mass and 2.3 percent muscle. Muscle made up 21.6 percent of everything lost, which the authors describe as comparable to the roughly 25 percent typically seen with calorie restriction alone. Blood pressure improved as well: systolic fell about 2.5 mm Hg and diastolic about 1.5 mm Hg.

The finding worth sitting with is the predictor. Baseline muscle-to-fat ratio was the strongest signal of both how much weight came off and what that weight was made of. Women who started leaner lost proportionally more muscle. As Chabassier's team frames it, averages hide large differences between individuals, and starting body composition flags who is most at risk.

Why this is an eating story, not a scale story

Here is the part the generic food lists miss. The medication already did the restricting. Appetite goes quiet, portions shrink on their own, and the risk stops being too much food and becomes too little of the right food. A GLP-1 eating plan is a protection plan: getting enough protein, fibre and fluid into an appetite that has gone very small. The 21.6 percent is a group average, not a forecast for you. If you began with more muscle relative to fat, this study says your personal share is likely higher.

What it means for you

Three plain moves, none of them dramatic.

Put protein first at every meal, before the vegetables and before the carbohydrate, because you may only manage half the plate. Most women on these drugs do best aiming for roughly 25 to 35g per meal. See why protein is the number that matters.

Lift something twice a week. Resistance work is the only signal that tells your body to hold on to muscle while fat comes off. Strength training on a GLP-1 covers the minimum version.
